Complex Care Specialist

Providing comprehensive care to individuals with complex health needs is a key aspect of this role. As part of a multidisciplinary team, you will work collaboratively to assess and coordinate client care, guiding them towards recovery and stability within a harm reduction framework.
Key Responsibilities- Conduct thorough assessments to identify clients' complex drug and alcohol issues
- Foster strong relationships with families and other services to facilitate effective treatment
- Work within a harm reduction framework to support clients with highly complex health and psychosocial needs, prioritizing their well-being and safety
- Extensive experience working with clients who have complex drug and alcohol issues, with a proven track record of delivering high-quality care
- Demonstrated commitment to working with Aboriginal clients and improving health outcomes for socially disadvantaged groups, promoting cultural sensitivity and awareness
- Experience in trauma-informed and harm reduction frameworks, crisis intervention, and care planning, with a focus on empowering clients to take control of their lives
Candidates will be required to demonstrate excellent communication and interpersonal skills, ability to work effectively as part of a multi-disciplinary team, and apply professional boundaries to ensure the highest level of care for clients.
