  • Assist participants with daily living skills, social and recreational activities, and accessing community services">
  • Implement and monitor strategies to promote a healthier lifestyle and empower participants to achieve their goals">
  • Work collaboratively with participants, families, and other stakeholders to ensure a seamless service delivery">

Requirements:

To be successful in this role, you will need:

  • 1 year of experience in Home Care (preferred)">
  • A current driver's license and reliable vehicle">
  • Working with Children Check and NDIS Screening Check (desired)">
  • First Aid Certification and Covid-19 vaccination">
