Complex Care and Frequent User Management Program

We are seeking a highly experienced Clinical Nurse Consultant to join our team in providing expert care and support to individuals with complex health needs.

The successful candidate will have a strong background in clinical nursing, advanced leadership skills, and the ability to work collaboratively with multidisciplinary teams. They will be responsible for developing and implementing personalized care plans, leading quality improvement initiatives, and providing education and training to staff and stakeholders.

  • Champion compassionate, innovative care strategies that reduce reliance on emergency services and improve outcomes for patients with complex health needs.
  • Design and implement personalized care plans that help patients confidently manage mental health, chronic pain, and substance use challenges outside of emergency settings.
  • Lead the growth and evaluation of the Complex Care and Frequent User Management Program, ensuring it meets evolving patient and system needs.
  • Collaborate with clinical teams and senior leaders to guide strategic initiatives that support better health outcomes and deliver impactful service improvements.

Key responsibilities include:

  • Providing expert clinical advice and guidance to multidisciplinary teams.
  • Developing and implementing evidence-based care plans and protocols.
  • Leading quality improvement initiatives and collaborating with other teams to achieve program goals.
  • Providing education and training to staff and stakeholders on best practices in complex care and frequent user management.

Requirements:

  • Current registration as a Registered Nurse with Authority to Practice from the Australian Health Practitioners Regulation Agency (AHPRA).
  • A minimum of seven years' full-time post-graduate experience, including five years' full-time equivalent in the specialty field.
  • Postgraduate nursing qualifications relevant to the specialty field, or evidence of working towards such qualifications.
