The HCP Care Partner role focuses on delivering outstanding person-centred support to clients, building meaningful relationships with internal and external stakeholders to ensure clients receive the care and support they need.

Key Responsibilities
  • Coordinate services within business guidelines
  • Implement client service responsibilities in line with client needs
  • Ensure Aged Care Quality Standards and compliance requirements are met
  • Conduct assessments and reviews as required, and undertake intake of new clients

The role offers flexibility with work-from-home options and travel to visit clients as required.

About You
  • You must be a supportive, friendly, and organised individual with excellent time management, leadership, and customer services skills
  • Experience in a similar role and relevant qualifications are essential
  • You have knowledge, understanding, and experience in aged care, disability, and/or Home and Community care
  • A genuine desire to deliver outstanding customer service is required
