Job Overview

The Disability Navigator is a critical role within the healthcare sector, responsible for providing care coordination and navigation services to individuals with disabilities.

This position requires a strong understanding of the National Disability Insurance Scheme (NDIS) and the ability to effectively communicate complex care plans to various stakeholders.

Key Responsibilities
  • Support NDIS-eligible individuals who are hospitalized or at risk of hospitalization by facilitating access to necessary services and supports.
  • Collaborate with multidisciplinary teams to develop and implement personalized care plans, ensuring successful discharges from hospital.
  • Navigate the disability sector by utilizing knowledge of alternative supports and services to optimize outcomes for clients.
Selection Criteria
  • A tertiary qualification in a relevant field, preferably Social Work or an Allied Health profession, with 3 years of clinical experience and registration eligibility with the relevant professional body.
  • Demonstrated expertise in advanced clinical reasoning, assessment, and treatment of people with disabilities.
  • Proven experience working in the disability sector, with a strong understanding of the NDIS and its related supports.
  • Ability to coordinate and communicate complex care plans, prioritize caseloads, and manage stressful situations autonomously.
  • A current unrestricted NSW drivers' license and willingness to travel across the region, occasionally staying overnight.
