Job Title: Dedicated Care Professional

About the Role:

  • Deliver holistic, client-centered care aligned with each client's tailored care plan.
  • Assist clients with daily living activities, ensuring their comfort and well-being in their own homes.

Key Responsibilities:

  • Provide personal care and assist with daily activities, following the Client Care Plan.
  • Collaborate closely with Registered Nurses to deliver client-specific care and address changing needs.
  • Identify client care requirements and work effectively with the client's care team to ensure seamless support.
  • Communicate clearly and empathetically with clients, their families, and team members.
  • Support clients with light housekeeping duties to promote independence and confidence.
  • Maintain accurate daily client notes and promptly report any concerns to the RN/Care Manager.

Essential Qualifications
  • Cert III in Aged Care/Disability (or equivalent).
  • Current Senior First Aid, CPR, and Manual Handling certificates.
  • A roadworthy and insured vehicle with a current driver's licence.
  • Federal Police Clearance.
  • Flexibility with shifts and willingness to travel as required.
  • Strong communication and interpersonal skills.
  • Knowledge of relevant legislation and standards for home care services.
