About the Opportunity

We are seeking a highly skilled and compassionate Care Coordinator to join our team at Uniting NSW.ACT. This is an exciting opportunity to work with young adults who are homeless or at risk of homelessness, providing them with the support and guidance they need to achieve their goals.

Job Summary

The Care Coordinator will be responsible for assessing the needs of clients and developing individualized care plans to address their unique challenges. This may involve providing emotional support, connecting clients with local resources, and advocating on their behalf to ensure they receive the assistance they require.

Key Responsibilities

  • Care Coordination: Develop and implement individualized care plans that meet the unique needs of each client.
  • Assessment and Planning: Conduct thorough assessments of clients' needs and develop care plans that address their specific challenges.
  • Client Support: Provide emotional support and connect clients with local resources to help them achieve their goals.
  • Advocacy: Advocate on behalf of clients to ensure they receive the assistance they require.
  • Documentation: Maintain accurate and up-to-date records of client interactions and progress.

Requirements

  • Cert IV in Community Services or related field
  • 2+ years of experience in youth work or homelessness support
  • Excellent communication and interpersonal skills
  • Able to work independently and as part of a team
  • Possess a strong understanding of best practices in Specialist Homelessness funded services
