An exciting opportunity to be involved in a dementia initiative that supports people living with dementia transitioning from hospital settings back to their homes or into residential care.
Key Responsibilities- Apply expertise in assessment and biopsychosocial approaches, implementing practical strategies to prevent, minimize, or respond to changes in behaviour related to dementia.
- Support families, caregivers, and clinical staff to understand how dementia impacts each individual.
- Equip carers and clinicians to prevent and respond to behaviour changes through coaching and development of resources including behaviour support planning.
- Develop reports that are contextualized to individual situations and people, effecting change on the ground and improving quality of life for people living with dementia.
- Expertise in supporting people living with dementia.
- Experience in residential aged care and/or community aged care.
- High level of negotiation, communication, and relationship building skills.
- Excellent problem-solving and decision-making skills.
