Role Overview

A dementia centre initiative supports individuals with dementia transitioning from hospital to home or residential care. This role offers career development and exposure to a new project.

The Dementia Support Coach will provide on-site support, implement relationship-based strategies, model and coach clients, carers, and care staff.

To achieve this goal, we need you to collaborate with peers to develop aged care staff and carer skills, expertise, and knowledge in understanding dementia and providing best practice support and care.

You will support the implementation of recommendation plans using modelling and coaching techniques, seeking guidance as required.

Furthermore, you will facilitate brokerage item trials with clients and carers and seek guidance as needed.
