Lead Support Specialist for Dementia Transition

We are seeking an experienced Lead Support Specialist who will play a pivotal role in supporting people living with dementia as they transition from hospital settings back to their homes or into residential care.

  • Facilitate on-the-ground support and implementation of relationship-based support strategies.
  • Model and coach clients, carers, and care staff.
  • Collaborate with peers to develop aged care staff and carer skills and expertise.

  • The ability to work independently and as part of a team.
  • Demonstrated commitment to customer service principles.
  • A current driver's licence and safe driving record.
  • Willingness to travel and work intrastate and interstate.

Key Qualifications: Relevant degree, experience working with older adults, strong communication skills, patient-focused approach, attention to detail, problem-solving skills, adaptability, and resilience.
