About the role:
Nurse practitioners play a pivotal role in delivering high-quality patient care. They possess enhanced skills and knowledge in assessment, diagnosis, and evaluation of nursing care.
As an Advanced Clinical Care Specialist, you will work independently to assess, diagnose, and manage patients' health needs. You will also provide specialist nursing interventions that demonstrate an advanced level of nursing knowledge and skills.
You will be responsible for developing and implementing management plans, providing advanced clinical support and leadership through evidence-based practice, and contributing to the health equity agenda.
Working with our multidisciplinary team, you will actively promote the elimination of racial discrimination and institutional racism, increase access to healthcare, influence social, cultural, and economic determinants of health, and deliver culturally safe and responsive health services.

Requirements:
- Postgraduate qualification in Nursing or related field
- Registered nurse with current practicing certificate
- Experience in primary health care and/or chronic disease management
