Salary and Benefits

Job Description

: As a Care Coordinator, you will be responsible for managing acute patient care, enhancing and promoting their quality of life. You will work across a 7-day roster, between the hours of 7am and 10pm, and will be required to utilise virtual technology to coordinate patients' treatment plans.

Your clinical skills, care planning, coordination skills, and intuitive people skills will be central to your success in this role. You will be knowledgeable about acute healthcare practice and bring strong clinical reasoning and assessment skills.

Required Skills and Qualifications

: To be successful in this role, you will require proficient computer skills to enable fast and accurate data entry into patient electronic medical records. Recent experience in acute care management in a hospital or community setting amongst a multidisciplinary team will be highly regarded.
