About the Role

An exciting opportunity has arisen for a skilled and experienced Registered Nurse to share the position of Clinical Care Coordinator - Cardiac unit at Calvary Adelaide Hospital.

This key role is responsible for coordinating complex discharge planning in accordance with health fund DRG's and desired length of stay, as well as undertaking cardiac discharge education and making appropriate referrals to outpatient rehab programmes.

The successful Registered Nurse will have a comprehensive knowledge and understanding of cardiac surgical and interventional procedures, as well as expected length of stay.

Responsibilities
  • Coordinate complex discharge planning in accordance with health fund DRGs and desired length of stay
  • Undertake cardiac discharge education
  • Making appropriate referrals to outpatient rehab programmes

The ideal candidate will be a registered nurse with a minimum of 2 years post-qualification acute experience, who wishes to develop skills in case management and discharge planning.

Requirements
  • Registered with APHRA, and with a minimum of 2 years post-graduate experience in acute care hospital nursing
  • Cardiac acute nursing experience is required
  • An understanding of health funds is highly desirable
  • Demonstrated advanced interpersonal and organisational skills and a high level of analytical, communication and reporting skills
  • Demonstrated effective working relationships with Medical Practitioners, Managers, and all staff
