About This Role

An exciting opportunity exists for a skilled Registered Nurse to share the position of Clinical Care Coordinator - Cardiac unit at Calvary Adelaide Hospital. The successful candidate will work in a job share capacity, with work days to be considered, and to cover annual leave as required.

Key Responsibilities
  • Coordinating complex discharge planning in accordance with health fund DRG's and desired length of stay
  • Undertaking cardiac discharge education and making appropriate referrals to outpatient rehab programmes
  • Working effectively with Visiting Medical Officers, Managers and Nursing and Allied Health teams
Requirements
  • Registered with APHRA, and with a minimum of 2 years post-qualification acute experience in acute care hospital nursing
  • Cardiac acute nursing experience is required
  • An understanding of health funds is highly desirable
  • Demonstrated advanced interpersonal and organisational skills, a high level of analytical, communication and reporting skills, and effective working relationships with Medical Practitioners, Managers, and all staff
