At Calvary Care, we are seeking an experienced Registered Nurse to fill the position of Advanced Cardiac Care Coordinator in our Adelaide Hospital. As a key member of our team, you will play a crucial role in coordinating complex discharge planning and ensuring that patients receive the highest quality care.
About the RoleThis is an exciting opportunity for a skilled and dedicated nurse to join our cardiac unit. You will be responsible for working closely with medical officers, managers, and other healthcare professionals to provide exceptional patient care.
Key Responsibilities- Coordinate complex discharge planning for cardiac patients, taking into account health fund DRGs and desired length of stay.
- Undertake cardiac discharge education and make referrals to outpatient rehab programs as needed.
- Develop effective working relationships with medical officers, managers, and nursing and allied health teams.
- Demonstrate advanced interpersonal and organisational skills, with a high level of analytical, communication, and reporting skills.
- Minimum 2 years post-qualification acute experience as a Registered Nurse.
- Cardiac acute nursing experience is essential.
- Understanding of health funds is highly desirable.
