Key Responsibilities
- Collaborate with Aboriginal clients and health professionals to provide integrated care services.
- Facilitate management of chronic disease(s) for clients with end-stage renal disease.
- Work closely with the team to enhance the health and wellbeing of Aboriginal clients.
Requirements
- Current registration with AHPRA as a registered nurse or registered Aboriginal Health Practitioner.
- At least 2 years' post-graduate work experience.
- Excellent understanding of primary health care and sound clinical knowledge of chronic disease management.
