Renal Care Coordinator Specialist

**Job Summary:**
We are seeking a highly skilled and dedicated Renal Care Coordinator to join our team at Central Australian Aboriginal Congress. As a key member of our care coordination team, you will work collaboratively with Aboriginal clients and health professionals to provide culturally safe and integrated care services.
**Key Responsibilities:
- Work closely with clients to develop and implement individualized care plans that address their complex chronic conditions.
- Collaborate with multidisciplinary teams to ensure comprehensive and coordinated care.
- Facilitate communication between clients, families, and healthcare providers to ensure seamless transitions and optimal outcomes.
**Requirements:**
- Current registration as a registered nurse or registered Aboriginal Health Practitioner with AHPRA.
- At least 2 years' post-graduate experience in primary health care and chronic disease management.
- Excellent understanding of Aboriginal health and community control principles.
