We are seeking a skilled Renal Care Coordinator to join our team at Central Australian Aboriginal Congress.

About the Role

This full-time, continuous contract position plays a crucial role in providing culturally safe care services for clients with complex chronic conditions of end-stage renal disease requiring haemodialysis.

The successful candidate will work collaboratively with Aboriginal clients and health professionals to enhance their health and wellbeing by facilitating management of their chronic disease(s).

Key Responsibilities
  • Deliver multidisciplinary and integrated care services to clients with complex chronic conditions.
  • Collaborate with health professionals to provide culturally safe care.
  • Facilitate management of chronic diseases to improve client outcomes.
Requirements
  • Current registration with AHPRA as a registered nurse or registered Aboriginal Health Practitioner with at least 2 years' post-graduate work experience.
  • Excellent understanding of primary health care and sound clinical knowledge of chronic disease management.
  • Understanding and commitment to Aboriginal health, and the philosophy and practice of Aboriginal Community Control.
