Renal Care Coordinator

Job Summary
The Renal Care Coordinator plays a vital role in providing culturally safe and integrated care services for clients with complex chronic conditions of end-stage renal disease requiring haemodialysis.
Key Responsibilities
- Collaborate with Aboriginal clients and health professionals to develop and implement individualized care plans.
- Provide education and support to clients and their families on chronic disease management and self-care.
- Work closely with the multidisciplinary team to ensure seamless care coordination and communication.
- Contribute to the development and implementation of policies and procedures that promote Aboriginal health and wellbeing.
Requirements
- Current registration with AHPRA as a registered nurse or registered Aboriginal Health Practitioner.
- At least 2 years' post-graduate work experience in a relevant field.
- Excellent understanding of primary health care and sound clinical knowledge of chronic disease management.
- Understanding and commitment to Aboriginal health, and the philosophy and practice of Aboriginal Community Control.
