Salary: The base salary for this position is $103,759 - $126,121 (p.a.), with a total effective package of $110,838 - $134,420 (p.a.).
Job Description: As a Renal Care Coordinator, you will work collaboratively with Aboriginal clients and health professionals to provide culturally safe multidisciplinary and integrated care services for clients with complex chronic conditions of end-stage renal disease requiring haemodialysis.
Role Responsibilities: Your primary goal will be to enhance the health and wellbeing of Aboriginal clients with significant chronic conditions and receiving dialysis by helping to facilitate management of their chronic disease(s).
Requirements: To be considered for this position, you will require current registration with AHPRA as a registered nurse or registered Aboriginal Health Practitioner with at least 2 years' post-graduate work experience. Additionally, you must have an excellent understanding of primary health care and sound clinical knowledge of chronic disease management.
