Job Description:

The Renal Care Coordinator plays a crucial role in providing culturally safe multidisciplinary care services for clients with complex chronic conditions requiring haemodialysis. The position aims to enhance the health and wellbeing of Aboriginal clients with significant chronic conditions by facilitating effective management of their disease(s).

About the Role:

This is a full-time, continuous contract position that requires collaboration with Aboriginal clients and health professionals to deliver high-quality patient care.

Key Responsibilities:

  • Providing individualized care plans and coordinating multidisciplinary team meetings
  • Culturally sensitive case management and support for Aboriginal clients
  • Facilitating communication between clients, families, and healthcare providers
  • Maintaining accurate records and reporting as required

Requirements:

  • Current registration with AHPRA as a registered nurse or registered Aboriginal Health Practitioner with at least 2 years' post-graduate work experience
  • Excellent understanding of primary health care and sound clinical knowledge of chronic disease management
  • Understanding and commitment to Aboriginal health, and the philosophy and practice of Aboriginal Community Control
