Clinical Care Specialist

The Renal Care Coordinator plays a vital role in enhancing the health and wellbeing of Aboriginal clients with complex chronic conditions requiring haemodialysis. This position aims to facilitate management of their chronic disease(s) by working collaboratively with Aboriginal clients and health professionals to provide culturally safe multidisciplinary and integrated care services.
Key Responsibilities:
- Providing clinical care to clients with complex chronic conditions
- Working collaboratively with Aboriginal clients and health professionals
- Facilitating management of chronic disease(s)
Requirements:
- Current registration with AHPRA as a registered nurse or registered Aboriginal Health Practitioner
- Excellent understanding of primary health care and sound clinical knowledge of chronic disease management
- Understanding and commitment to Aboriginal health and the philosophy and practice of Aboriginal Community Control
