Chronic and Complex Care Specialist

We are seeking a skilled professional to join our interdisciplinary Connected Care team, providing comprehensive assessments, care coordination, linkages to services and specialist allied health and medical care for clients with chronic and complex conditions in the community.
The ideal candidate will possess strong communication and interpersonal skills, with the ability to work collaboratively as part of a dynamic team. A high level of autonomy is required, as well as the capacity to prioritise tasks effectively and manage multiple projects simultaneously.
Key Responsibilities- Conduct thorough assessments of client needs and develop tailored care plans
- Collaborate with multidisciplinary teams to ensure seamless care delivery
- Establish and maintain effective relationships with clients, carers and other stakeholders
- Stay up-to-date with best practice and evidence-based approaches to care coordination and management
