**About the facility**
This role will be part of the North Area Health Child and Family Health Services Team, located at Barwon Health North and provides community based intervention. Barwon Health North provides accessible healthcare close to home and to public transport bridging the gap between the acute settings at the University Hospital Geelong by providing the following services:
- Urgent Care Services
- Medical Imaging
- Child and Family Services
- Healthy at Home (Chronic and Complex Care)
- Dialysis Specialist Clinics
**About the role**
This role which has been established as part of the Child and Family Health Services team; which is based in the Northern Suburbs of Geelong. The role forms part of a broader north area health team, focusing on delivering high quality services and as part of the team, linking into the range of services delivered both from Barwon Health North and locally in our community.
The complex family services navigator role aims to facilitate the patient and family journey in what has become an increasingly complex health and service provider system. The purpose of this role will be to reduce fragmentation, mitigating barriers, educate, empower and coordinate care. The key principles related to the complex family services navigator role are: Coordination of complex care systems navigation in particular with ECEI and NDIS providers, creating partnerships, improving outcomes and systems improvement for the families accessing the Child & Family Health Services Team at Barwon Health North.
