Community Care Specialist

We are seeking a highly skilled and dedicated Community Care Specialist to join our interprofessional team. In this role, you will be working closely with clients with chronic and complex conditions, providing comprehensive assessments, care coordination, linkages to services, and specialist allied health and medical care.
