About the Role

We are seeking an experienced Support Coordinator to join our team at Community Solutions Group. As a Support Coordinator, you will play a vital role in helping NDIS participants achieve greater independence by assessing their individual needs and identifying the necessary services and supports to reach their goals.

Key Responsibilities
  • Build Strong Relationships: Develop and maintain strong relationships with participants, their families, and external service providers to ensure effective coordination of supports.
  • Coordinate Supports: Work closely with participants to coordinate their supports, ensure they maximize their NDIS plans, and address any barriers by implementing action plans.
  • Drive Business Growth: Identify opportunities to increase business growth and revenue through effective service delivery and participant engagement.
  • Manage Service Agreements: Ensure service agreements and service bookings are completed in a timely and efficient manner.
  • Capacity Building: Take a holistic approach to capacity building and help participants to break down and address their barriers.
Requirements
  • NDIS Knowledge: A thorough understanding of the NDIS and the ability to understand facets of NDIS Plans/Budgets.
  • Administration Experience: Plenty of administration experience, compliance capabilities, and IT skills - particularly with Excel.
  • Business Acumen: A business mindset and a customer service focus.
  • Time Management: Excellent time management and prioritisation skills with high-level organisation skills.
