**About the Role**

As the Service Manager for the Commonwealth funded Transition Care Programme (TCP), you will play a pivotal role in leading and managing a dynamic, multidisciplinary aged care service. You will be responsible for the strategic direction, policy development, and contract management of TCP-related aged care services within the Sub-Acute Directorate. Your leadership will be crucial in implementing key frameworks, managing strategic projects, and fostering strong relationships with Commonwealth and State Departments, Primary Health Networks, and aged care providers across residential and community settings.

This role offers an exciting opportunity to contribute to the strategic planning and development of aged care services within CALHN. You will work as part of a collaborative leadership team, driving quality development initiatives, managing contractual obligations, and overseeing the financial accountability of the programme. Your impact will extend to building strong links with consumers, community stakeholders, and other service providers, ensuring the successful delivery of high-quality care to older Australians transitioning from hospital to home.

**About You**

We are looking for a dynamic and experienced leader with the following key skills and qualifications:

- Strong leadership ability in a complex, multidisciplinary environment with proven experience in change management and service delivery improvement.
- Expertise in developing, implementing, and evaluating health service programmes, with a commitment to quality improvement and performance monitoring.
- Demonstrated experience in policy development, effective communication, and fostering interagency relationships.
- Sound knowledge of the aged care sector, health care funding models, and occupational health and safety principles.
