**Selection Criteria**:

- 1. Highly developed communication and interpersonal skills and an ability to maintain comprehensive data and accurate case notes on students
- 2. Demonstrated ability to undertake individual support, counselling and risk assessments where appropriate for students presenting with various wellbeing and inclusion needs (i.e. self harm/Anxiety/Intellectual Disability/ASD/ADHD etc.)
- 3. Demonstrated ability to develop and review appropriate documents to support the individual and the school based on identified need of student (i.e. wellbeing and/or inclusion needs)
- 4. Demonstrated ability to support school wellbeing policy and to provide direction and recommendations to school staff and families in order to support the wellbeing and inclusion needs of students
- 5. Demonstrated ability to develop and maintain networks that enhance the provision of effective internal and external services for students at risk

**Role**:
The purpose of this role is to coordinate and support inclusion and wellbeing structures as well as practices for the Camberwell High School community. Camberwell High School¿s approach to inclusion and wellbeing recognises the best learning environment and the best educational outcomes are achieved through the shared partnership between students, parents and teachers.

The Inclusion and Wellbeing Counsellor will have a prime focus across the Junior School and Senior School, will work within the Wellbeing and Inclusion team and will also report directly to the Assistant Principal liaising with the Wellbeing Team.

**Actions/Tasks**:

- Provide counselling and support to students
- Provide support to staff where appropriate
- Lead the development and assist in the delivery of a variety of programs that promote and improve student wellbeing, disability inclusion, safety, resilience, engagement, and connectedness, across the school
- Lead the implementation of the Student Engagement and Wellbeing Policy in consultation with sub-schools
- Collaborate with Student Learning Leaders, House Leaders and classroom teachers to develop individual support plans for identified students to address their personal needs
- Provide information and advice to parents and students upon request, including coordination and referral to DET Student Support Services, other Government agencies and alternative education/training/employment programs
- Assist Sub-School Leaders, Student Learning Leaders, House Leaders, and the Careers staff with the transition of new students to school and when exiting CHS to alternative pathways
- Provide information and feedback to the Sub-School Leaders regarding individual student¿s needs, issues and concerns and assist in the development of effective learning programs that can be implemented for students at risk
- Provide weekly and confidential feedback and updates to the Principal Class about students and staff being assisted
- Participate in meetings of local and/or networks of school wellbeing staff as are seen to be appropriate
- Supporting the annual process of year 6
- 7 transition
- Upload documentation to PSDMS
- Support the wellbeing and inclusion team in the administration of learning assessments (i.e. PAT and YARC)
- Manage the annual process of NCCD data collection
- Schedule assessments and organise room booking for Assessment Australia and DET Student Support Services testing
- Lead PSD Educational Needs Questionnaire parent meeting with DET Nominee

Liaise with external stakeholders and services for PSD funded students
- Manage SOCS database and process of getting consent form signed
- Provide support to teaching staff to familiarize them with the Inclusion referral process
- Oversee the delegation and development of Special Needs Report (SNR) for Inclusion students
- Oversee the development and management of Individual Learning Plans (ILP) for Inclusion Students
