Role Summary
We are seeking an experienced professional to support the delivery of culturally safe palliative care services to Aboriginal people.
The successful applicant will work closely with clinicians, community-based service providers and patient families to ensure respectful and compassionate end-of-life care.
About the Role
This is an Identified Aboriginal/Torres Strait Islander Position requiring applicants to be of Aboriginal descent through parentage, identification as Aboriginal and acceptance in the community.
The ideal candidate will possess a strong understanding of issues affecting Aboriginal people in relation to grief and loss, as well as Aboriginal social and emotional wellbeing.
Main Responsibilities
- Support the development of culturally safe environments for Aboriginal patients and their families within a multidisciplinary team.
- Collaborate with clinicians and community-based service providers to provide comprehensive care coordination.
- Act as a conduit for Aboriginal patients with palliative care needs in both hospital and community settings.
- Work across the district to integrate culturally safe practices in care coordination.
Requirements
- Holds a current NSW Driver's Licence.
