About the Opportunity

  • At ECH, we believe that effective care coordination is the backbone of a wellfunctioning healthcare system. As the Manager of Care Coordination, you will play a vital role in ensuring seamless transitions of care and optimizing client experiences. You will have the chance to lead a dynamic team, working together to enhance patient outcomes and improve the overall quality of care.
  • You will ensure service delivery is compliant with Aged Care Quality Standards, procedures, policies and supports best outcomes for clients. With a proven track record of developing and maintaining excellent relationships with internal and external stakeholders along with exceptional team management skills, you will lead the provision of care coordination that is responsive client's changing circumstances as they age, recover or transition, in line with ECH service Guidelines
  • This includes managing client feedback, integrating this into continuous process improvements.
  • You will lead, manage, and mentor the Care Coordination team ensuring they have the required skills, knowledge, and capacity to fulfill the requirements of their role, including identifying any training needs of the care coordination team.
    About You


You will have the relevant tertiary qualification or extensive experience in Human services, Allied Health, Age Care or Community Service Fields.

You have knowledge and/or experience in Home Care Packages.

You are kind, even when a bit stressed out, dynamic and energetic. You are also flexible and adaptable to change.
