**Role Overview**:
This is an exciting opportunity to join UnitingSA’s team as the Team Leader, Care Coordination. This role provides leadership to the Community Aged Care (CAC) team, Care Coordinators, and ensures the efficient coordination of community based aged care client care and support in alignment with a range of program requirements.

Working in partnership with clients and stakeholders, the Team Leader, Care Coordination is accountable for ensuring our care coordination team deliver a quality and professional service to our clients that supports and promotes their independence, enablement and wellness through the coordination of assessed needs based services and support, maintaining allocated HCP budgets and CHSP service delivery.

This role enables the delivery of a Great Client Experience that ensures compliance with the Aged Care Quality Standards and client service agreements.

**Key Responsibilities**:

- Build a team culture that promotes accountability, inclusion, and delivery of a Great Client Experience through every interaction.
- Provide leadership to the Care Coordination team in a manner that ensures engagement and high levels of performance through ongoing performance coaching, feedback and development.
- Ensure recruitment of individuals who have good cultural alignment to UnitingSA’s Values and facilitate retention of key talent.
- Build a team culture that embraces a continuous improvement mindset that enhances the client experience of people accessing our services and the people experience of our Care Coordinators.
- Proactively promote Great Client Experience, ensuring clients receive the highest standard of care and service provision that enables the client to indecently live in their homes and community.
- Monitor the delivery of quality care through workplace observations, client feedback and stakeholder information ensuring that diverse and often complex needs of clients are met.
- Provision of effective leadership with the ability to provide role clarity and ongoing feedback to direct reports that enables them to deliver person-centred care in line with Care Plan/s.
- Ensure that client support/care is consistent with UnitingSA service delivery model.

**Skills & Experience**:

- Demonstrated experience leading, developing and building a diverse team of professional Care Coordinators.
- Experience in demonstrating change leadership and the ability to effectively communicate with and influence others toward acceptance and understanding of change.
- Demonstrated prior experience in identifying, scoping, planning and implementing continuous improvement initiatives that support great service delivery and client experience.
- Demonstrated ability to identify gaps in service delivery and client care and work collaboratively with key stakeholders to resolve operational challenges in a highly constructive manner.
- Prior experience working in Community Aged Care / Home Care (Desirable).
- Knowledge of the Community Aged Care / Home Care Sector and Accreditation process (desirable)
- Knowledge of legislation applicable to Aged Care Service delivery (desirable)
- Ability to work with a high degree of initiative, judgment and responsibility over the Care Coordination Team, taking only high-level direction from Senior Leaders.
