**About the Opportunity**
- ECH Care Coordinators are responsive to their clients ever changing needs. Under the guidance of our operational and clinical service leadership team, our Care Coordinators:
- Undertake comprehensive assessments of client needs to maximise their HCP budget and to ensure individualised goals are developed into a Care Plan that enable them to continue to live independently within the community.
- Accountable for the care coordination of a delegated caseload. Ensuring compliance requirements are maintained and client care plans reflect the current state of their care needs.
- Ensuring a KPI of 85% utilisation of client budget is met
- Identify when a client’s needs have changed to ensure they continue to receive appropriate care and advocate to support their increasing needs.
