We are seeking a Clinical Care Coordinator to join our team in Adelaide CBD. As a Clinical Care Coordinator, you will be responsible for coordinating clinical services to support clients in their own homes.

You will be the primary point of contact for clients and staff, ensuring services are delivered efficiently and effectively.

Your key responsibilities will include:

  • Coordinating daily care for new and existing clients of the SA Health Community Care Program.
  • Accepting referrals and ensuring all client information is available for service delivery.
  • Identifying clients eligible for other Government funding streams and liaising with referrers and healthcare professionals.

About You

We are looking for a highly skilled and experienced Clinical Care Coordinator who possesses:

  • A Bachelor of Nursing degree.
  • Experience in acute nursing and community nursing.
  • A full driver's license.
  • Previous experience managing service contracts and staff.
  • Strong problem-solving and time management skills.
  • Effective communication skills.
  • Computer skills.
  • Ability to work over 7 days a roster.
