We are seeking a skilled Clinical Care Coordinator to join our team in Adelaide CBD. As a key member of our team, you will be responsible for coordinating clinical services to support clients in their own homes.
Key Responsibilities- Coordinate daily care for new and existing clients of the SA Health Community Care Program.
- Accept referrals, including the Metropolitan Referral Unit (MRU), and ensure all client information is available for service delivery within required timeframes.
- Identify clients eligible for other Government funding streams and liaise with referrers, MRU, or healthcare professionals for care transfer.
- Bachelor of Nursing
- Experience in Acute Nursing
- Community nursing experience
- Full Driver's Licence
- Previous experience managing service contracts
- Previous experience managing staff
- Strong problem-solving and time management skills
- Effective communication skills
- Computer skills
- Ability to work over 7-day roster
- Logistical experience
