We are looking for a permanent **Care Manager - Home Care Packages** to join our caring and dynamic office team in Adelaide. Just Better Care Adelaide Care Managers play a key role in helping families receive the care and support services they need.
- Our Care Manager is responsible for the development of individualised support plans in consultation with clients, their family or advocates and key health professionals. This role ensures effective delivery of professional nursing care and support to clients, respecting client’s rights in consultation and cooperation with the client, their advocates, their medical practitioner and health professionals.

**Duties and responsibilities include**:
- Undertakes business development activities and supports the development and implementation of local area marketing initiatives.
- Provides prospective clients with relevant and comprehensive information regarding Just Better Care services.
- Ensures appropriate information is provided and explained to the client at all times regarding the service agreement, budget and the client handbook.
- Provides support advisory services to clients and undertakes comprehensive client assessment and planning by assessing, monitoring and reviewing the needs of the client.
- Provide financial and case management services to customers, and representatives as required.
- Where applicable, develop strategies to address the identified needs of the client in consultation with their representatives where appropriate and in accordance with funding guidelines.
- Supports with staff recruitment, staff development and provision of staff training and skills assessments as required.
- Demonstrates a commitment to best practice, ensuring that all services are provided in a caring and respectful way in accordance with Just Better Care’s policy and procedures, and legislative requirements.
- Provides and delivers individualized and personalized support to clients (as required) in accordance with their care plans and/or medical practitioner’s directions.
- Responsibilities may include working under the guidance and supervision of medical practitioners, working with allied health professionals, monitoring and communicating the client’s condition, and other health and wellness related activities.
- **Current National Registration as a Registered or Enrolled Nurse**
- **Experience managing Home Care Packages**
- Intermediate computer literacy and data entry skills
- NDIS Worker Screening Clearance / Criminal Record Check
- Previous experience in the community care or health/aged care sector
- Current Driver’s Licence, First Aid Certificate/CPR
- Emotionally intelligent and an ability to empathise with customers
