About the role

Based at our Eden's Landing Community office you will work collaboratively with our elderly clients in the Logan area, assisting them to engage in services that enable them to remain living in their home. Your client's Care Plans will be developed in close consultation with your clients and their families ensuring the client's needs and wishes are met, whilst also ensuring strategies and services are engaged to optimise their health and independence. Your genuine passion for the elderly to live their best lives motivates you to strive for the best outcomes for your clients, simultaneously empowering them to achieve their individual goals and improving their quality of life.

You will be required to:

  • Collaborate with client and family to establish goals that are realistic and achievable
  • Develop, implement and evaluate strategies and services that satisfy clients' needs
  • Maintain client documentation
  • Collaborate with relevant persons to ensure that client's physical, social, emotional and clinical needs are met; including liaison with medical professionals as required
  • Analyse the engagement of each client with SCCQ and suppliers
  • Participate in the "On Call" system
  • Develop support systems that encourage the involvement of relatives, friends and community to the extent defined by the client
  • Coordinate / Case Manage care services for client within budget targets
  • Ensure Workplace Health and Safety processes are followed in accordance with SCCQ policies, procedures and processes
  • Complete other tasks as required

What you will bring to the role?

  • Qualifications in Nursing or Allied Health (desired)
  • Current Senior First Aid and CPR (desired)
  • Extensive experience with complex care or support management
  • Strong knowledge of DSS Home Care Consumer Directed Care
  • Strong conflict resolution and negotiation skills
  • Ability to manage time effectively and prioritisation skills
  • Excellent problem-solving skills with a solutions focus
  • Previous experience working within Sales targets, upselling and cross selling
  • Exceptional interpersonal skills and the ability to relate to a variety of people
  • Good phone manner including the ability to develop rapport with clients
  • Demonstrated ability to work autonomously
  • Intermediate skills in Microsoft Office

Southern Cross Care Queensland (SCCQ) is a Catholic not-for-profit organisation established by the Knights of the Southern Cross, more than 40 years ago. They saw a need to provide compassionate care and inclusive service to some of the most vulnerable, the ageing population, in the community.
