Position Overview:

We are seeking a highly skilled and experienced Disabilities Services Officer to join our dynamic team. This role is critical in providing person-centred care and support to individuals with complex needs, including those with challenging behaviors, high physical support requirements, and mental health issues. The successful candidate will possess exceptional interpersonal skills, a deep understanding of the NDIS framework, and the ability to coordinate and deliver comprehensive care in line with National Disability Standards.

Key Responsibilities:

  • Service Delivery: Provide high-quality personal and home care, adhering strictly to each individual’s Health and Support Plans, and maintaining a disciplined approach to service provision.
  • Person-Centred Support: Engage in tasks and activities that enhance the lives of individuals, promoting their participation and contribution to the community.
  • Team Collaboration: Work closely with service team members in the assessment, planning, implementation, and evaluation of individual care programs, ensuring seamless coordination and delivery.
  • Compliance: Deliver services in strict accordance with National Disability Standards, organizational policies, and procedures, ensuring the highest standards of care.
  • Skill Development: Support participants in developing skills necessary for improved living within the community, tailoring approaches to individual needs.
  • Behavioral Management: Implement internal personal support plans for participants with challenging behaviors and complex needs, utilizing your experience in mental health and physical support.
  • Record Keeping: Maintain meticulous records of client progress, services provided, and any incidents or changes, ensuring that all documentation is accurate and up-to-date. 
  • Coordination: Act as a key liaison between service providers, healthcare professionals, and community organizations to coordinate comprehensive care for clients.

Qualifications & Experience:

  • Successful completion of the DSC Support Coordination Course.
  • Current Police Check and Blue Card.
  • Valid Australian Drivers License.
  • Current First Aid and CPR Certificate.
  • A relevant qualification in Disability Services, Social Work, Community Services, or a related field.
  • A minimum of three years of relevant experience in a similar role may substitute for the formal qualifications listed above. Relevant on-the-job training and experience may also be required in addition to formal qualifications.

Skills & Competencies:

  • Superior verbal and written communication skills, with a demonstrated ability to handle challenging situations with professionalism.
  • Strong proficiency in Excel, Word, and NetHunt CRM systems.
  • Proven experience in managing mental health issues, complex behaviors, and high physical support needs.
  • Excellent phone communication skills with a client-focused approach.
  • Fast Typing Skills: The ability to type quickly and accurately is essential for this role, as you will be responsible for minute-taking during meetings, ensuring that all key points and decisions are documented efficiently.
  • Meeting Facilitation: Experience in conducting and facilitating staff meetings is required. You will be expected to lead meetings, ensure effective communication, and coordinate with team members to address any issues or updates related to client care and service delivery.
