You will support carers and participants living with a disability to receive the services and support required to maintain their wellbeing so they can live independently and achieve their goals. This position is a full-time position based in Mulgrave reporting to the Team Leader – Support Coordination.
About you
You will be an experienced passionate Support Coordinator with outstanding communication and client service skills allowing you to build strong relationships with a diverse range of people. You will have excellent time management and organisation skills with the ability to manage competing demands to meet client needs.
Requirements of the role:
Relevant industry experience and tertiary qualifications in Disability or Community Services Knowledge of NDIS, Disability Act and other relevant legislationYour responsibilities will include:
Providing advice and coordination services to carers / participants to empower choice and self-management to ensure the required services are received Plan and implement services and supports that empower participants to participate in activities. Promote best practice ‘person centred care’ Provide Carer support to address the wellbeing of the carer; introduce carer support services Maintain networks and knowledge of services and liaise with service providers.
