Why this role matters

The Salvation Army Australia’s Homelessness Stream provides high quality, person centred services to people at risk, and or experiencing homelessness. We support adults and families, including accompanying children with accommodation, case management, outreach support, financial assistance, connection and referral to other specialist services.

About the role

We are seeking a values-driven Case Manager who will provide case management support, in a community-based setting, that assists people to achieve housing stability, experience improved social inclusion and achieve a level of self-mastery that will enable them to sustain tenancy and lead their best lives.

This role is focused on providing specialised support to families accessing our mobile support services.

This support is achieved through person- and family-centred case management incorporating child inclusive, recovery, trauma-informed and strengths-based practices whilst working effectively with other members of the team to provide a comprehensive service that reflects the vision, mission and values of The Salvation Army.

You will successfully

  • Establishing relationships with individuals and families that access the service that encourage self-determination and participation
  • Providing case management support in accordance with TSA case management models and standards of practice
  • Ability to complete thorough assessments, inclusive of needs, risks and vulnerabilities.
  • Ongoing assessment of the individuals’ and family’s presenting issues, vulnerabilities and needs from initial intake to exit
  • Individual case planning in collaboration with the individual/family including a range of interventions that address the needs or goals of the individual, as well as barriers to achieving those.

Family Focused

This role will provide specialised support to families accessing our mobile support services, acknowledging that this cohort has a range of specific needs and challenges. You will need to prioritise providing support to families experiencing any, or all, of the following:

  • Child Safety involvement or complex legal custody arrangements
  • Domestic and Family Violence (particularly high-risk)
  • Household makeup of numerous dependents with complex needs.

This role would address the observed need for family specialised support and supports capacity of our broader mobile support service provision.

You will have

  • Tertiary qualification in Community Services, Social Work or related field .A degree level qualification is preferred.
  • Relevant experience in client assessment and case management is a must
  • QLD Working with Children Check – WWCC is mandatory -position is located at a site that provides overnight accommodation to single women .
  • Driver’s Licence
  • First Aid Certificate

This is a Permanent, Full-Time position located in Spring Hill, QLD, however a majority of this work is mobile support around the Brisbane areas . Salary and conditions are in accordance with the Social, Community, Home Care and Disability Services CRISIS Level 2 Award.
