Why this role matters

The Salvation Army Australia’s Homelessness Stream provides high quality, person centred services to people at risk, and or experiencing homelessness. We support adults and families, including accompanying children with accommodation, case management, outreach support, financial assistance, connection and referral to other specialist services.

About the role

We are seeking a values driven Case Manager who will provide case management support in a community setting, supporting individual to secure housing, experience improved social inclusion and achieve housing stability, this support is achieved through person-centred case management incorporating resilience, trauma informed and strengths-based practices whilst working effectively with other members of the team to provide a comprehensive service that reflects the vision, mission and values of The Salvation Army. This role is within the Homelessness Stream of the Social Mission Department.

You will successfully
- Establishing relationships with individuals that access the service that encourage self-determination and participation
- Providing case management support to individuals in accordance with TSA case management models and standards of practice
- Ability to complete thorough assessments, inclusive of needs, risks and vulnerabilities
- Ongoing assessment of the individuals’ presenting issues, vulnerabilities and needs from initial intake to exit
- Individual case planning in collaboration with the individual including a range of interventions that address the needs or goals of the individual, as well as barriers to achieving those
- Assisting individuals to access support services and establish community supports, and where needed provide advocacy for the person with service providers
- Regularly reviewing progress and outcomes with the person of individual case plans and where needed taking action to ensure the plan continues to meet the persons needs or goals
- Identifying risks to individuals, staff and TSA and addressing where appropriate of bringing to the attention of the Team Leader as soon as possible
- Service gaps are identified and recommendations for service delivery improvements are made to the Team Leader
- Participates in local network meetings inclusive of Communities of Practice meetings
- Displays high levels of emotional intelligence, with an awareness of how personal values and beliefs can impact work

This is a Max term full
- time position, based can be based anywhere in Queensland with our office based in Spring Hill. Salary and conditions are in accordance with the SCHADS Level 4.

You will have
- Tertiary qualification in Community Services, Social Work or related field. A degree level qualification is preferred.
- Relevant experience in a social service environment including in client assessment and case management
- A national police record check is required
- QLD Working with Children Check (position is located at a site that provides overnight accommodation to young people and/or provides support to families in a home setting)
- Compulsory Code of Conduct, WHS and Bullying and Harassment training modules are completed and up to date
- Driver’s Licence
- First Aid Certificate
