**About the Opportunity**
The Practice Coach is responsible for leading the learning and development strategies and coaching activities, that builds the service capacity for our Home and Community Care staff. This role will actively support the workflow and ensure daily practice is in accordance of the operation model of care, with person-centred principles.
They will be working alongside the leadership teams across the region, as well as the frontline staff, providing advice and support.
**About you**
- Tertiary or Diploma qualification in a relevant field, such as Aged Care, Health, Business and/or working towards or equivalent years of experience
- Certificate IV in Training and Assessment will be highly regarded
- Experience in the Aged Care and/or the Health environment
- Significant experience in coaching and supporting employees, ensuring content is updated and fit for purpose
- Ability to convey information and ideas through creative channels that will support understanding and the retention of information
- Confident in navigating and nurturing teams through ambiguity and change
- Passion for social change and contributing to an organisation of influence for vulnerable, at risk and disadvantaged members of the community
- Current driver's licence and access to a comprehensively insured vehicle
