**About the Role**
- We are looking for a Program Coordinator to be the primary point of contact for IRT customers. Reporting to the Program Team Manager the successful applicant will assist in ensuring our Commonwealth Home Support (CHSP) and Private Fee for Service (PFFS) customers are continuing to live a great life in their home with their individual goals supported by exceptional services. This role is based in Wollongong.
Responsibilities will include:
Communicate, develop and maintain relationships with customers enquiring or receiving CHSP and PFFS services
Liaise internally with IRT's Customer Contact Centre Care to assist with managing CHSP and PFFS referrals and in-taking new customer
- s Identify support needs of new/existing customers and coordinate referrals Maintain processes, documentation and evidence of interaction
- s Manage workloads to ensure designated deadlines are met Liaising with rostering team for service schedulin

- s Demonstrated experience in professional relationship building skills with the ability to manage expectations and credibilit

- y Demonstrated proficiency using computer system
- s Desirable: Certificate III in Individual Support or other administration qualification
- s Desirable: Experience in customer Care Plans and assessmen
