About the opportunity

The Stroke Coordinator is responsible for the promotion and operation of Werribee Mercy Hospital's (WMH) Stroke Service. This includes the day-to-day coordination and care of stroke and Transient Ischaemic Attack (TIA) patients who present to WMH. The role identifies and manages patients within the service throughout the course of their presentation.

The Stroke Coordinator:

- Ensures the efficient and effective provision of patient care, based on clinical care standards and best practice principles within a collaborative multidisciplinary team
- Participates in the coordination of person-centred, holistic care
- Supports excellence in clinical practice and promotes the development of the multidisciplinary team, within their scope of practice
- Participates in local, service, and subregional quality activities
- Demonstrates competence in the provision of person-centred care as specified by the registering authority's licence to practise, education preparation and context of care
- Demonstrates a sound level of skills in assessment, care planning, practice and evaluation in the provision of care to clients and their significant others

To thrive in this role, you will have:
Mandatory:
- Physiotherapist or occupational therapist holding registration to practice with AHPRA or speech pathologist with eligibility for practising membership of their professional association- Relevant tertiary qualifications (Bachelor, Masters or equivalent recognised degree)- Extensive clinical experience working with patients who have experienced a stroke- Demonstrated understanding of evidence-based stroke care- Experience working in acute, sub-acute, community, and rehabilitation settings- A current Victorian Driver's licence- A current Police Check dated within 12 months of issue

Desirable:
- Post-graduate qualification in a relevant clinical speciality- Specific high-level specialist knowledge in Stroke and TIA care- Strong analytical, problem-solving skills, and strategic thinking capability- Proven ability to complete administrative and reporting requirements to a high standard within set timeframes- Demonstrated ability to work autonomously, and proven ability to lead, co-ordinate, and work with a multidisciplinary team- Outstanding interpersonal and communication skills, with ability to communicate and negotiate effectively with stakeholders at different levels in health organisations- Experience in developing, delivering, or co-ordinating education programs for health professionals in a clinical environment- Experience in leading or participating in research activities- Highly developed organisational, written and interpersonal communication skills, including the ability to relate positively to people from diverse backgrounds- Demonstrated ability to be actively involved in CQI (Continuous Quality Improvement)- Experience in networking, coordinating, and liaising with a wide range of service providers and other relevant bodies

You will also be required to provide evidence of, or in the process of obtaining:
- A current Police Record Check and Working with Children Check.- Current COVID-19 certificate, proof of current influenza vaccination and other vaccination status consistent with public health orders.- Current drivers license.
