**Our client services**

Our client Home Modifications and Maintenance Services (HMMS) is offered for the elderly, people with disabilities, and carers of adults and children with a disability to allow them to remain in their own home. Services range from minor maintenance tasks to more complex modifications such as widening access ways and installation of access ramps and full bathroom replacement and renovations.

**Role Overview**
- Managing referrals from start to finish
- Ensuring the accurate recording of client information to allow for effective completion of required works
- Delegating work received through My Aged Care, NDIS referrals and other sources of new client business (e.g., Occupational Therapist referrals, Community Housing, other external sources)
- Ensuring office processes are streamlined and followed
- Processing Accounts Payable and Accounts Receivable, meeting deadlines including end of month and end of financial year
- Maintenance of financial records relevant to client cases
- Submission of collected information to support funding body requirements
- Telephone support
- Processing of new incoming referrals (Intake functions)
- Assessing eligibility of client referrals
- Liaising with contractors and other service providers as required
- Maintaining client files and filing systems, both papers based and electronically
- Receipt and dispatch of mail
- Support progress towards service delivery targets through contribution to an effective and efficient office
- Liaising with workshop and contractors
- Scheduling jobs and staff to meet operational goals
- Providing other general support to achieve Home Modification and Maintenance goals as directed by service manager time to time.

**Role Responsibilities**
- To Manage referrals made to HMMS & Property Maintenance through My Aged Care, NDIS, and other funded sources
- To ensure client records are accurate to support effective client management and reporting
- To ensure clients accessing the funded services of HMMS & Property Maintenance receive the correct services
- To provide timely advice to ensure service delivery targets are met for our clients
- To support the effective transfer of information between the various areas of HMMS & Property Maintenance
- To support reporting requirements of HMMS & Property Maintenance, both internally and externally
- To provide a comprehensive, coordinated and integrated range of basic maintenance and support services for frail aged people, people with a disability who in the absence of basic maintenance and support services are at risk of premature or inappropriate admission to long-term residential care
- To support flexible, timely services that respond to the needs of clients.
- Be a strong ambassador for our client Home Modifications and Maintenance Service

**Performance Measures**
- Client referrals are actioned in a timely manner
- Meet client's satisfaction
- Clients referred to HMMS & Property Maintenance receive the services they require
- High communication skills with subcontractors equipment and material suppliers

**Essential Criteria**
- Strong coordination and planning skills
- Strong and clear communication skills both written and verbal
- Ability to manage multiple projects simultaneously
- Good computer literacy, particularly in database maintenance/entry
- An ability to set priorities and to achieve them
- Ability to meet company's KPIs
