Access Support Coordination (ASC) are specialists in providing their clients with the right access to supports to accelerate their independence and achieve their goals.
**About this opportunity**:
The Support Coordinator is responsible for connecting participants with a NDIS Plan to formal and informal services and supports in their community and assisting participants in understanding and reviewing their NDIS plan.
**Key responsibilities of this role**:
- Build the capacity of the participants to achieve greater independence to self-direct services and support in the long term
- Ensure mainstream services meet their obligations and organisational responsibility
- Provide the participants and the NDIA with reports on outcomes and success indicators, within the agreed reporting frequency
- Oversee the implementation of service agreements for participants including funding capacity
- Assist participants to understand service options and to make decisions in relation to preferred options
- Link participants to service providers, and support participants to obtain quotes and establish service agreements with selected providers
- Ensure evidence is collated to improve client outcomes
- Develop relationships with key community stakeholders & liaise with other statutory bodies and government departments
- Investigate suitable options for service users in the way of recreational, career orientation or accommodation options
**To succeed in this role you will have**:
- Experience in a Support Coordination role or similar for a minimum of 2 years (specifically with mental health is highly desirable)
- Certificate IV in Disability or other relevant qualification (highly desirable)
- Strong knowledge of local disability community and providers
- An understanding of billable hours in relation to KPI targets
- Basic accounting and budgeting skills
- Excellent written and verbal communication skills
- Working with Children Blue Card and NDIS Worker Screening/Yellow Card
- Drivers Licence
