Role Summary

This role involves managing and coordinating ongoing care for consumers/patients who are patients of concern within a hospital setting.

The successful candidate will provide specialist nursing advice, consultation, leadership, and education to inter-professional team members as relevant to the specific context of the role.

Key Responsibilities
  • Practice in accordance with code of ethics, professional standards, and legislation affecting nursing practice by maintaining current knowledge and competence within scope of position.
  • Provide leadership and direction within the hospital which facilitates a consistently high level of nursing care.
  • Contribute to the health equity agenda by actively supporting the elimination of racial discrimination and institutional racism, and supporting increased access to healthcare, influencing social, cultural, and economic determinants of health, supporting delivery of sustainable, culturally safe and responsive health services, and recognizing the importance of working with Aboriginal and Torres Strait Islander peoples, communities, and organizations to design, deliver, monitor, and review the health and support services provided.
Required Skills and Qualifications

To be successful in this role, you will need to have:

  • Advanced practice skills and experience in nursing coordination and management.
  • Strong leadership and communication skills.
  • Ability to work effectively in a team environment.
  • Knowledge of health equity principles and practices.
