– Goodna, QLDPart-Time or Full-Time | SCHADS Award Level Based on Qualifications & ExperienceAbout the RoleThe Base Support Services Inc. (TBSSINC) is seeking a professional and experiencedNDIS Support Coordinatorto join our team with the option for an immediate start.
This role focuses on delivering high-quality, person-centered support coordination to NDIS participants, including those with complex needs.
The position is hands-on and doesnot involve supervision or management of other staff.The successful candidate will be responsible for working directly with participants to implement their NDIS plans, coordinate formal and informal supports, and ensure services align with the goals and preferences of each individual.
A strong working knowledge of NDIS legislation, stakeholder engagement, and compliance reporting is required.Key ResponsibilitiesCoordinate and implement NDIS plans, supporting participants to achieve their goals and exercise choice and control.Assist participants in managing their plan budgets, including interpreting service categories and tracking utilisation.Navigate legislative reporting obligations and ensure compliance with the NDIS Quality and Safeguarding Framework.Engage with complex participants and collaborate with multidisciplinary stakeholders including justice liaisons, cultural liaison workers, allied health, and community service providers.Support participants subject to QCAT mattersEngage with the Office of the Public Guardian and the Public Trustee Queensland in relation to participant decision-making, financial management, or legal mattersAdvocate for participant rights and promote supported decision-making and the dignity of risk.Maintain accurate case notes, participant files, and progress reports in line with legislative and organisational standards.Use Brevity Care Software to record and manage participant data, case notes, and reporting efficiently and accurately.Essential Criteria2-4 years' experience in NDIS Support Coordination service deliveryTertiary qualification in Human Services, Social Work, or related field (determines wage level under SCHADS Award).
Applicants with Support Coordination training and demonstrated experience will also be considered.Strong understanding of the NDIS framework, participant budget management, and service coordination requirements.Demonstrated experience supporting participants with complex needs, including those involved with statutory bodies.Understanding of legislative requirements related to QCATExperience engaging with the Office of the Public Guardian and the Public Trustee Queensland in relation to participant decision-making, financial management, or legal mattersHigh-level communication and interpersonal skills to effectively liaise with a wide range of stakeholders.Competent in the use of Brevity Care Software or similar client and case management systems.Excellent time management and the ability to work independently while maintaining service quality.Other RequirementsCurrent Queensland Driver's License.Current Blue Card (Working with Children Check) and Disability Worker Screening - Yellow Card Exemption (or ability to obtain).
