**Role Overview**

The Case Coordinator will play a vital role in coordinating transitional and community-based care for younger patients (0-65 years) who are medically ready for discharge or require hospital avoidance strategies. You will act as the central point of contact between clients, families, hospital teams, and NDIS providers to ensure seamless care transitions, reduced hospital stays, and improved quality of life for clients, including those with complex behaviours of concern (BoC).

**Key Responsibilities**
- Conduct holistic assessments and develop person-centred care plans tailored to individual needs.
- Coordinate transitional supports including accommodation, daily living assistance, community access, and disability-related health supports.
- Liaise with hospital teams, NDIS planners, allied health professionals, and families to facilitate timely discharge and avoid unnecessary hospital admissions.
- Provide intensive case coordination to navigate NDIS access, plan approvals, and plan reviews.
- Ensure culturally safe and trauma-informed practices, particularly for First Nations clients.
- Monitor, review, and adjust care plans to align with client goals, funding availability, and sustainable long-term outcomes.
- Maintain accurate documentation, reporting, and compliance with NurseLink's governance, safety, and quality systems.

**Qualifications & Skills**
- Tertiary qualification in Social Work, Nursing, Allied Health, Community Services, or related field.
- Demonstrated experience in case management, discharge planning, or NDIS service coordination.
- Strong understanding of hospital avoidance strategies, disability services, and transitional care.
- Excellent communication, advocacy, and stakeholder engagement skills.
- Ability to manage complex behaviours of concern with a compassionate, person-centred approach.
- Knowledge of NDIS processes, community care systems, and culturally safe service delivery.
- Proficiency in client management systems and Microsoft Office Suite.
