Role Summary

">
  • Deliver tailored case management services for young people at risk of disconnecting with their family/community, disengagement from education/training/employment, homelessness, or at risk of harm.

Key Responsibilities:

">
  • Engage effectively with young people, their families and support network, identifying needs and aspirations, and recording and progressing a plan created and led by the young person.
  • Act as the main 'point-of-contact' for the young person and their family, working in partnership to facilitate supports required to achieve their goals.
  • Develop, implement and execute the care plan of young people, coordinating a wrap-around suite of services to achieve outcomes of four program objectives.
  • Assist young people to realise increased self-worth, family and community connectedness, and safety, delivering a service underpinned by customer service principles.

Essential Skills and Qualifications:

">
  • Bachelor's Degree in social work, human services, social science, health, allied health, youth work, community services, family intervention, or relevant years' experience in field.
  • Minimum of three years' experience in case management and coordination activities using a client information system.
  • Demonstrated ability in working with multiple agencies providing access to holistic care for clients.
  • Demonstrated ability to meet targets and performance outcomes.
  • Knowledge of supports available to young people in areas of mental health, education, employment and housing.
  • Ability to communicate sensitively and effectively to young people through decisive listening, influencing and persuasive skills.
  • Current drivers licence.
  • Willingness to travel across Brisbane region.
  • Willingness to complete 1-2 night outreach shifts a week.
  • Working with Children's Card (Blue Card) or eligibility to apply.
