Primary Care Physician
Deliver exceptional primary healthcare services to young people as part of a collaborative and holistic team that values diversity and inclusivity.
Responsibilities:
Conduct thorough assessments for individuals experiencing physical and mental health issues, providing high-quality care that meets their needs.
Collaborate with internal and external stakeholders to provide comprehensive care, ensuring seamless transitions and optimal outcomes.
Develop Mental Health Care Plans for internal referrals and Allied Health Services, fostering a supportive environment for patients to thrive.
Provide accessible and inclusive primary healthcare services to young people, addressing their unique health concerns and promoting overall well-being.
Respond to medical/health concerns presented by patients, including diagnosis, investigation, treatment, and referral as appropriate, prioritizing patient-centered care.
Encourage young people to engage with sexual health and drug and alcohol services, as well as preventative healthcare initiatives, promoting healthy lifestyles and disease prevention.
Have access to clinical supervision from the clinical manager, ensuring ongoing professional development and support.
Essential Criteria:
Approved tertiary qualifications in a relevant discipline, with current registration with AHPRA, demonstrating expertise and commitment to the field.
Strong clinical experience in youth health and mental health services, showcasing ability to navigate complex cases and deliver effective interventions.
Proven ability to apply evidence-based practices in therapeutic settings, promoting best practices and optimizing patient outcomes.
Experience in coordinating care for young people with diverse health needs, highlighting capacity to manage multiple priorities and ensure comprehensive support.
Capacity to work effectively within a multidisciplinary team, fostering collaboration and driving positive change.
